Your child will not be able to begin care unless all the indicated forms below are filled out and returned
on or before the first day of care - including a copy of his/her immunization record NO EXCEPTIONS.
Your contract and policy sheets have been provided for you to fill out and bring back with the additional
forms below. If you have any questions please do not hesitate to ask.
